The Transitions of Care Program through ComForCare Home Care provides clients with one-time care services or continued care plans, which include a nurse’s evaluation of needs, a personalized routine based on the evaluation and home safety checks as needed. ComForCare Home Care can help to transfer your loved one from a medical facility, such as a hospital, to an assisted living community or to their home in the Mountainside area.

Short-Term Assistance

With hospital-to-home transition care, our caregivers are able to provide transportation assistance, help an individual with meal preparation, errands, following discharge plans, light housekeeping and more.
