Our Caregivers Have Advanced DementiaWise® Training to Support People with Alzheimer’s and Other Forms of Dementia
Our mission is to help families and individuals in Southeast Michigan cope with the onset of Dementia and help manage long-term effects. We understand the unique challenges Dementia presents to families with loved ones experiencing the hurdles the condition brings. Our caregivers are DementiaWise certified and have received specialized training to help keep loved ones engaged in life so that that can continue to create new and meaningful memories with their families.
Symptoms of Dementia are as varied as the people it affects. No two persons will endure the same thing, and care plans need to reflect that.
At ComForCare (Washtenaw, MI), we take a one-on-one strategy to Dementia care to tailor a unique care plan for your loved one. Our strategically crafted plans are structured to identify focus areas, set goals, and build routines and mental exercise regiments that improve a patient’s quality of life.
We partner with clients, their families, and their medical professionals to make the best decisions regarding ongoing care. Our caregivers work on your schedule to provide the personalized care your loved one needs.
We begin our dementia care services by administering an initial in-home evaluation to start creating a care plan. It includes working one-on-one with our clients to learn their distinct needs completely. During this phase, we attempt to learn, among other things:
After our evaluation, we will work with you and your loved one’s other healthcare professionals to craft a comprehensive, personalized care plan. Our plans are flexible and, through information gathered from closely monitoring the client’s progress over time, can be updated to serve their needs as they evolve.
We also communicate with family members to understand their needs regarding care for their loved ones. Our goal is to match the right caregiver with your loved one to meet their needs and provide them the support they need on their terms.
We know the emotions associated with watching someone suffering from Dementia. That is why we are committed to helping families understand, manage, and improve the quality of their loved ones’ lives. We believe that trained caregivers and personalized care plans can help bring positive change to our friends and neighbors in our community.
Our Dementia care services are also available for short-term periods to allow families to recuperate and recharge. We know first-hand how challenging caring for a loved one can be, and families must have a chance to step back without feeling like their loved ones are being neglected. When you need a break, you can feel comfortable knowing that ComForCare will lend a hand in your time of need.
