Home Care is one of the essential services Provided by Medicaid, CO

Through Prior Authorization Requests (PAR) Medicaid recipients are provided Medicaid benefits that include home care services like meal preparation, showing assistance, light housekeeping, laundry and more. Health First Colorado requires that Medicaid recipients receive a waiver for homemaker (companion/personal care) which covers health maintenance activities. 

It also covers Remote Support, also known as Remote Patient Monitoring, which ComForCare is able to provide. Remote support provided in combination with home care provides you 24/7 coverage with no out of pocket expense to you.

How to get started

The focus is always on services that are flexible, scheduled based on need and approval.

  1. Contact ComForCare to start: 303-515-6755
  2. From there, we’ll reach out to your case manager notifying them of the care request and learn more about eligibility and coverage. 
  3. Next, a member of our team will meet with the member first to discuss your needs.
  4. Finally, an assessment will be completed and will be submitted on your behalf for authorization since home care services require a PAR, prior authorization request, and start of care date.
  5. Lastly, we will begin services.

  • If you already have a Case Manager: You most likely already have an active waiver. You can contact them directly to confirm your benefits.
  • If you do not have a Case Manager: You will need to apply. Reach out to your local county health and human services department and let them know you want to apply for Home and Community-Based Services (HCBS).
