What Is the GUIDE Medicare Program?
GUIDE stands for “Guiding an Improved Dementia Experience.” It’s an eight-year voluntary pilot program launched by the Centers for Medicare & Medicaid Services (CMS) to better support people living with dementia and their unpaid family caregivers.
Historically, Medicare hasn’t covered non-medical home care services—things like help with bathing, dressing, or temporary supervision while a caregiver takes a much-needed break. The GUIDE model changes that. If you qualify, Medicare may now pay up to $2,500 annually for in-home respite care, provided by credentialed agencies like ComForCare.
This is a major shift in how the U.S. approaches health care for individuals with dementia. By supporting families at home, this program helps people avoid premature placement in long-term nursing homes and maximize quality of life.
Who Is Eligible for GUIDE?
To qualify for the GUIDE program’s respite care benefit:
- Your loved one must be diagnosed with dementia or Alzheimer’s
- The person must be enrolled in Medicare
- They must receive care from a provider participating in the GUIDE model (like ComForCare)
- You must be providing ongoing support as an unpaid caregiver
