Somerset County Homemaker–Home Health Aide Program

We’re honored to be a contracted provider for the Somerset County Homemaker–Home Health Aide (HHHA) Program, administered through the Somerset County Board of Social Services. This program helps eligible residents remain independent by delivering essential home care services at little to no cost—a compassionate alternative to institutional care for those who need a hand to keep living safely at home Somerset County+1.

To participate, individuals must apply through the Board of Social Services, where eligibility is determined by both medical need (verified via a physician’s note confirming the need for assistance with activities of daily living) and financial need (assessed based on income and assets) Somerset County.

Once approved, clients receive a tailored care plan developed via a social assessment, with up to 20 hours per week of homemaker–home health aide services, depending on their specific circumstances. Services include help with personal care tasks like bathing, dressing, mobility, transferring, feeding, meal prep, light housekeeping, and laundry Somerset County.

Typically, a modest monthly cost share may be required, calculated based on income and living arrangement, while the county covers the remaining cost of the service Somerset County. Our role is to provide trained, bonded, and insured Certified Home Health Aides who deliver compassionate, person-centered care. We coordinate closely with the county’s social workers to ensure continuity, comfort, and dignity for each client’s unique needs Somerset County.
