We can assist one time, or occasionally as needed. For example, our loved one may need help when being discharged from the hospital. When called on, we will coordinate pick up with a discharge planner, a social worker, or the nursing staff at a hospital or rehab facility. Our caregiver will follow the patient-specific plan, pick up prescriptions, and get your loved one comfortably settled in at their home, or other living facility.

We can make phone calls to the family with progress reports and updates as requested or needed. Working as a team, we can ensure adherence to the discharge plan while keeping the client comfortable, thereby reducing or eliminating the need for readmission.

Continuing to monitor care after release from a facility is crucial for a full recovery. ComForCare is here to help clients who require extra supervision and assistance after a hospital stay or procedure, including around-the-clock care. With short term care, our caregivers can help a person recovering from a surgery or illness. We also provide transportation, assist with meal preparation, help run errands, help with light housekeeping, and more.

And most important, your loved one is not alone; a caregiver will be there if changes in health or behavior occur, and be able help or get help, whatever the situation. Our dedicated caregiver will provide companionship, medication reminders, and safety checks. Peace of mind is yours when a ComForCare is at your side.

We understand that sometimes the reason for a lengthy hospital stay is due to something that affects the individual’s ability to perform daily activities during recovery. Most people would prefer to get home, and have someone come in to help, rather than lie in a hospital. In this situation, ComForCare can develop an extended care plan, including around-the-clock care, to help meet your loved one’s needs so they can return home sooner -and recover at home.

Your loved one may be living with a chronic condition that requires on-going care. With 24/7 care including overnight, we can manage any condition. We always monitor and update care plans as necessary. Our team stays in constant communication with you and your family to decide what is beneficial for your loved one. With ComForCare on your team, you are never alone.

Whether your loved one needs assistance only a few hours a week or around-the-clock, our team is happy to help! Call (610) 628-9705 to learn more about the transition care services offered through ComForCare Home Care (Lehigh Valley, PA).
