Compassionate Transitions of Care in Ohio
At ComForCare we have processes in place to help people transition home after a stay in a hospital, a rehabilitation center, or skilled nursing facility. You can count on our experience and expertise.
Depending on circumstances, care needs can vary and change. For that reason, we offer Transitions of Care: one-day, short-term, and long-term care. We help you to successfully and safely return home after being away for treatment, hospitalization, or any medical reasons. We offer a “Transition to Home” package, and a “Preparing for Discharge” package. We can provide as much or as little support as you request to provide a safe return home, including picking up prescriptions, hospital discharge instructions, and more. A strong support system at home, along with our coordinated care, can reduce one’s risk of hospital readmission. If continued care is required, a nurse’s evaluation and a personalized routine can be developed, based on the assessment.

ComForCare's exclusive DementiaWise® program brings caregivers to your door who are specially trained to engage and enhance the lives of those with Alzheimer's disease and other forms of dementia while providing support and education for your family. The DementiaWise® program implements proven practices for dementia care, follows strategies to recognize behavioral triggers, and take special care to decrease challenging dementia behaviors.
Creating a personalized daily routine and setting goals is another method that directs a person with dementia toward positive emotional experiences. We encourage family members to have a schedule for health purposes, too. Our caregiver's presence offers respite for family caregivers to run errands, take a break, or take a nap. You must take time for yourself and rejuvenate.
Our caregivers are trained to guide your loved ones to engage in meaningful activities. The Joyful Memories® program creates connections in older adults using the power of music. Research shows that the areas of the brain that store musical memories are the last to deteriorate. Music often inspires those who rarely speak to sing. It encourages movement and brings out those who are becoming withdrawn.
With ComForCare at your side, you can expect your loved one to have better days, and you will both enjoy more time together.
ComForCare's DementiaWise® is recognized by the Alzheimer's Association® for incorporating evidenced-based Dementia Care Practice Recommendations in the following topic areas: Alzheimer's and dementia, person-centered care, assessment, and care planning, activities of daily living, and behaviors and communication.
