The Transitions of Care Program through ComForCare Home Care provides clients with one-time care assistance or develops and updates, as needed, care plans. Our nurses are able to evaluate needs and follow a client’s discharge plan. This can include developing a personalized routine based on the plan provided and conducting home safety checks as needed, which helps ensure a safe living environment. ComForCare Home Care can help to transfer your loved one from a medical facility to an assisted living community or to their home in the southern Denver area.

Short-Term Assistance

With hospital-to-home transition care, our caregivers can provide transportation services and assist with meal preparation, running errands, medication and appointment reminders, household chores, and more. What Is Transitional Care?

Transitional care is when someone moves from one care setting – such as a hospital, skilled nursing facility, rehabilitation center or even their home – to another. Unfortunately, these transitions don’t always go smoothly and a person can be readmitted to the hospital.

For individuals that need Transitional Care services, ComForCare offers a one-time “Transition to Home” package. This includes assistance from the hospital, rehabilitation center or skilled nursing facility along with help getting settled in at home. We’ll even call a friend or family member to let them know their loved one has arrived home.

Preparing for Discharge
