Bring Your Loved One Home Safely with Transitional Care

Your transition support plan should include professional care from ComForCare.

Transitional Home Health Care

One-Time Assistance

The Transitions of Care Program offered by ComForCare North Wake County provides clients with one-time care assistance. Our nurses are able to evaluate needs and follow a client’s discharge plan. This can include developing a personalized routine based on the plan provided and conducting home safety checks as needed, which ensures a safe living environment. ComForCare can assist with transporting your loved one from a medical facility to an assisted living community or to their home in northern Wake County, North Carolina.

Short-Term Assistance

With hospital-to-home transition care, our caregivers can provide transportation services and assist with meal preparation, running errands, medication and appointment reminders, household chores and clients’ self-care education. Our team members are happy to help your loved ones because we understand it’s important to provide individuals with supervision and assistance after a hospital discharge in order to ensure their safety and well-being. For older adults, continuing to monitor care after release from a facility is crucial for a full recovery. If an individual requires around-the-clock care following a hospital discharge, ComForCare is happy to provide a helping hand.

Ongoing Assistance

We understand sometimes the reason for a hospital stay is due to something that affects the individual’s ability to perform everyday activities during their recovery period. In this situation, ComForCare can develop and update, as needed, a care plan to help meet your loved one’s needs on a continual basis.

Our transitions of care services in northern Wake County, North Carolina can include medication reminders, condition monitoring, transportation, family respite, companionship and personal care. With ComForCare at your side, you can rest assured knowing your loved ones will be receiving the best care possible from our highly qualified caregivers.

What Is Transitional Care?

Transitional care is when someone moves from one care setting – such as a hospital, skilled nursing facility, rehabilitation center or even their home – to another. Unfortunately, these transitions don’t always go smoothly and a person can be readmitted to the hospital.

Transitional care services from ComForCare Home Care are available 24 hours a day, seven days a week - including holidays. Services vary by location.

Transition to Home Package

For individuals that need Transitional Care services, ComForCare offers a one-time “Transition to Home” package. This includes assistance from the hospital, rehabilitation center or skilled nursing facility along with help getting settled in at home. We’ll even call a friend or family member to let them know their loved one has arrived home.

Preparing for Discharge

As you or a loved one prepare for discharge, call us for a no obligation consultation. We can provide as much or as little support needed to help someone safely transition home. Our caregivers are available 24/7 including holidays, and we customize our care based on each client’s needs and preferences.
