Sometimes it is just one time, or on occasion. Life is not always predictable. In the case of an early hospital discharge, or emergency surgery, family members cannot always be there for their loved one.

Sometimes families don’t live in the same city or state. There are so many times that ComForCare can help!

When called on, we are able to coordinate pick up with a discharge planner, social worker, or the nursing staff at a hospital or rehab facility. Our caregivers will follow the patient-specific plan for at home care, pick up prescriptions, and get your loved one comfortably settled.

One-Time Care

We can assist one time, or occasionally as needed. For example, we can assist in transferring your loved one from a medical facility to an assisted living community or to their home. Working as a team, we can ensure adherence to the discharge plan while keeping the client comfortable, thus reducing or eliminating the need for readmission.

Short-Term Care

Continuing to monitor care after release from a facility is crucial for a full recovery. ComForCare is able to help clients who require extra supervision and assistance after a hospital discharge, including around-the-clock care if needed. With hospital-to-home transition care, our caregivers provide transportation, assist with meal preparation, help run errands, follow discharge plans from medical facilities, set medication reminders, help with light housekeeping and more.

What Is Transitional Care?

Transitional care is when someone moves from one care setting – such as a hospital, skilled nursing facility, rehabilitation center or even their home – to another. Unfortunately, these transitions don’t always go smoothly and a person can be readmitted to the hospital.

Transitional care services from ComForCare Home Care are available 24 hours a day, seven days a week - including holidays. Services vary by location.

Transition to Home Package

For individuals that need Transitional Care services, ComForCare offers a one-time “Transition to Home” package. This includes assistance from the hospital, rehabilitation center or skilled nursing facility along with help getting settled in at home. We’ll even call a friend or family member to let them know their loved one has arrived home.

Preparing for Discharge

As you or a loved one prepare for discharge, call us for a no obligation consultation. We can provide as much or as little support needed to help someone safely transition home.
