When you choose transition of care services through ComForCare, you can rest easy knowing that your family will receive the highest level of care and support from our trained and compassionate caregivers.

We recognize that falls can be dangerous for those undergoing a transition of care. A bad fall can start a person down a path of further health complications. This is especially important when moving a client into a new location where dangers may not have been previously addressed. We focus specifically on preventing falls by removing obstacles and identifying ways to provide your loved one with safer mobility during any transitional care plan.

One-Time Hospital-to-Home Care Services

Thankfully, our staff are experts in safely guiding clients on the next step of their healing journey.

When your loved one begins a transition of care, our team will meet with medical personnel, review discharge procedures, pick up prescriptions, and safely get your loved one home. Caregivers will then review the discharge plan with your family, answering any questions you may have.

Transitional Care Services Include:

  • An experienced, reliable caregiver who meets your loved one in the hospital room or discharge area
  • Transportation home with stops for picking up prescriptions or groceries
  • Help understanding and following discharge plans
  • A debriefing phone call to the family (or doctor or health coach)
  • Scheduling and attending follow up appointments
  • Meal preparation
  • Light housekeeping and laundry
  • Identify trip hazards and make home safety recommendations
  • Educate the client and family members on red flags or warning signs for chronic conditions
  • Bathing, grooming, and personal hygiene

Short-Term Care Services

Caregiver providing senior assistance to a wheelchairContinuing to monitor care after release from a hospital or other medical facility is crucial for a full recovery. ComForCare (Lanham, MD) is there to help persons who require extra supervision and assistance after a hospital stay, including around-the-clock care.

Working with families in Bowie/Lanham, Upper Marlboro, and Beltsville, we ensure adherence to the discharge plan or other medical instructions while keeping a loved one comfortable. In-home care reduces anxiety and helps clients recover faster, reducing or eliminating the need for a re-admission.

Long-Term Care Services

Young woman painting senior woman's nailsWe understand that a lengthy hospital stay often affects an individual’s ability to perform daily activities during recovery. For example, hip surgery can have a person off their feet for weeks and increase the risk of re-injury and re-hospitalization. ComForCare knows that being in a familiar and comforting environment can help speed the recovery process when facing a long and potentially painful rehabilitation period. That is why we have developed a transition of care model that monitors and updates care plans as conditions change, adapting to our clients’ recovery process.

Whether your loved one needs assistance only a few hours a week or around-the-clock, our team is happy to help! Call (240) 764-5844 to learn more about the transition care services offered through ComForCare Home Care (Prince George’s County, MD).

What Is Transitional Care?

Transitional care is when someone moves from one care setting – such as a hospital, skilled nursing facility, rehabilitation center or even their home – to another. Unfortunately, these transitions don’t always go smoothly and a person can be readmitted to the hospital.

Transitional care services from ComForCare Home Care are available 24 hours a day, seven days a week - including holidays. Services vary by location.

Transition to Home Package

For individuals that need Transitional Care services, ComForCare offers a one-time “Transition to Home” package. This includes assistance from the hospital, rehabilitation center or skilled nursing facility along with help getting settled in at home. We’ll even call a friend or family member to let them know their loved one has arrived home.

Preparing for Discharge

As you or a loved one prepare for discharge, call us for a no obligation consultation. We can provide as much or as little support needed to help someone safely transition home. Our caregivers are available 24/7 including holidays, and we customize our care based on each client’s needs and preferences. Contact a local office to learn more about transitional care services in your area.
