Why Choose ComForCare?
ComForCare Home Care was established in 1996 as a premier provider of private duty, non-medical home care with over 200 branches spanning the entire United States. ComForCare Home Care of Jacksonville has earned the reputation as a premier and professional in-home health care provider because we provide an unparalleled quality of care.
ComForCare maintains extremely high and specific standards regarding the in-home health care that we provide. We are licensed by the State of Florida, Agency for Health Care Administration (AHCA) with a Home Health Agency and Home Maker and Companion license. We are meticulous in our selection of the most highly qualified, responsible and compassionate caregivers. We only employ home health caregivers whom we would feel comfortable sending into our own parent’s homes.
The In-Home Health Care Process
To begin the application process, the ComForCare Home Care Admission Service Agreement is completed, and then a registered nurse (RN) from our agency meets with the prospective client/family. During that first meeting the nurse writes a comprehensive assessment that outlines the care plan and specific client needs. A copy of the assessment stays in the client’s home and serves as a guide for the ComForCare caregivers. The RN provides case management by staying in touch with the client, the responsible party, and the caregivers. The RN makes regular home visits to the client to reassess needs and levels of care.
