Caring for someone living with dementia can bring difficult questions, changing responsibilities, and a need for dependable support. ComForCare Home Care North San Diego is an approved provider for GUIDE respite services. We help local families understand the Medicare GUIDE Model, begin the application process, connect with a participating GUIDE organization, and coordinate personalized in-home respite care when approved.
Our local team serves families throughout North Inland, North Coastal, and East San Diego County.
The Guiding an Improved Dementia Experience (GUIDE) Model is a voluntary Medicare program designed to improve care and quality of life for people living with dementia while providing additional support for their unpaid caregivers.
Through participating GUIDE organizations, eligible individuals can receive coordinated dementia care, care navigation, caregiver education, and access to a care team. Qualifying families may also receive covered respite services to give unpaid caregivers time to rest and manage other responsibilities.

The GUIDE Model is designed for eligible people living with dementia who have Original Medicare. Families can contact our local team to begin exploring eligibility and available services.
| Medicare Coverage

Participants must have dementia. The participating GUIDE organization reviews diagnosis and other eligibility requirements. |
