Transitions of Care
ComForCare Home Care (North Austin)

Transitions of Care

We provide transitions of care services to individuals depending on their needs. Transitions of care can include one-time care, short-term care, or long-term care. According to a study done by the National Institute of Health (NIH), transitions of care play a very important role in reducing unnecessary re-hospitalizations and worsening of illness. Our transition care process is client-centered, focusing on your loved one’s safety, continuity of care, and collaboratively making a care plan that fits their needs.

One-Time Care

We can assist in transferring your loved one from a medical facility to senior living, rehabilitation centers, or to their home. We can also work closely with the discharging teams, adhere to discharge recommendations, schedule necessary follow-up appointments, and make sure all the medical equipment is in place to create a smooth transition of care.
