ComForCare takes a client-centered approach to home care. We meet each potential new client in their home, prior to beginning home care services. Our caregivers work closely with patients and family members to determine scheduling and care needs. Family members can take as little or as much control over caregiving as desired, with ComForCare caregivers filling in the gaps or taking charge as needed.
In the initial visit, a registered nurse evaluates the home for safety as well as the potential client’s condition to make recommendations for the care plan. The ComForCare team carefully selects a caregiver or caregivers based on personality similarities and expected compatibility, and the care plan and schedule are crafted. The family and the client and the ComForCare staff become a team to move forward with the best care plan possible.
Our experienced caregivers are available to assist with mobility around the house, hobbies, and attending social events. They can play board games, read books and magazines, or they can just sit back and chat.
It’s important for family caregivers to have the time they need to rest and recharge in order to stay healthy. When family caregivers need temporary relief, we can provide outstanding care for your loved one and peace of mind for you.
In-home care needs can vary depending on circumstances, and for that reason, we offer Transitions of Care. We offer one-day, short-term, and long-term care. We will decide together how much care you need and for how long. When continued care is required, a nurse’s evaluation and a personalized routine can be included, based on the evaluation.
Our DementiaWise® program engages and enhances the lives of the individuals with dementia while providing support and education for the family. We also have a music program we call Joyful Memories® that does wonders for uplifting spirits, memory recall and reaching those who have become withdrawn.
Alzheimer’s disease is among the most common form of dementia in the senior population. Depending on the stage, seniors with Alzheimer’s experience a range of physical, mental, and emotional changes. In advanced cases, seniors with Alzheimer’s will require assistance with activities of daily living. We help to relieve families of worry and stress by offering extensive Alzheimer’s care services.
