Meet the Owner, Lydia Gibson, DNP, CRNP, FNP-BC

Mission Statement: To provide compassionate, high-quality, and patient-centered home health services that support individuals across the lifespan, with a specialized focus on senior care. Guided by the clinical expertise of a board-certified Family Nurse Practitioner, we are committed to promoting independence, enhancing quality of life, and delivering personalized care that meets the physical, emotional, and social needs of each client in the comfort of their home.

We strive to improve health outcomes and empower individuals and families by delivering evidence-based, culturally sensitive, and coordinated care that allows clients- especially older adults- to age safely, comfortably, and with dignity in their homes.

Founded in 2025, ComForCare Home Care South New Jersey and Media, PA was established by Lydia Gibson, DNP, CRNP, FNP-BC, a board-certified Family Nurse Practitioner with extensive experience providing compassionate, high-quality care to individuals across the lifespan. With a strong clinical background and a deep commitment to patient-centered care, she has created the agency to address the growing need for reliable, personalized home care services- particularly for older adults and individuals with complex care needs.

Throughout her nursing and advanced practice career, Lydia has cared for diverse populations in a variety of healthcare settings, developing specialized expertise in senior care, chronic disease management, preventative care, and care coordination. Her firsthand experience working closely with patients and families revealed the critical importance of developing skilled, respectful care in the comfort of one’s home.

Guided by her clinical knowledge and passion for advocacy, Lydia founded ComForCare Home Care South New Jersey and Media, PA with a mission to enhance quality of life, promote independence, and support the physical, emotional, and social well-being of every client served. The agency is built on a foundation of professionalism, integrity, and compassion, ensuring that care is tailored to each individual’s unique needs while maintaining the highest standards of clinical excellence.

• 20 years of nursing experience which includes 15 years as a Registered Nurse and 5 years as a Family Nurse Practitioner

• Designation in Best Practices in Dementia Care and Caregiver Support by the Center to Advance Palliative Care (CAPC)

• Check for potential fall risks with suggestions for simple fixes
• Look at the living space with your loved one's specific concerns in mind
• Review medical history, medications, any other needs or concerns
• Evaluate balance and ease of movement throughout the home
• Recommend any necessary home safety devices that assist in overall comfort and safety such as handrails and shower chairs
• Offer support and understanding, with flexible services that can help
