Patient-Centered Care

Help your loved one live their best life possible with ComForCare care services.

Most home care plans start with a list of tasks. Patient centered care South Jersey families choose us for starts somewhere different: with the person. Before a single task gets assigned, we learn your loved one’s routine, their preferences, how they like to spend a morning, what makes them comfortable, and build the plan around that, not the other way around.

What Patient Centered Care Actually Means

Patient centered care is an approach, not a specific service. It means the person receiving care has a real voice in how that care is delivered, what time they’d rather bathe, which caregiver personality actually fits them, how involved they want family members to be in decisions. A generic home care plan applies the same routine to everyone. A patient centered one starts by asking what this particular person actually needs and wants.

How We Build a Patient Centered Care Plan

Every plan starts the same way: a free in-home visit where we sit with your family, not just to assess physical needs, but to understand your loved one as a person. We ask about their typical day, their history, their preferences, and their goals, then design a care plan around that picture. The services involved, whether that’s personal care, companionship, or respite support, are chosen to fit the person, not the other way around.

Dignity, Choice, and Control in South Jersey

For many seniors, losing a say in their own daily routine feels like a bigger loss than the physical need for help itself. Patient centered care protects against that. Your loved one keeps choosing when they eat, what they wear, how they spend their afternoon, with a caregiver supporting those choices rather than replacing them.

Adapting as Needs Change

A patient centered plan isn’t fixed once it’s written. As your loved one’s health, mobility, or preferences shift, we revisit the plan with them, not just about them. For families managing multiple providers or a more complex situation, our senior and elder care management service helps keep that coordination consistent over time.

Patient Centered Care for Memory Loss

This approach matters most when memory loss is involved. A patient centered plan for someone living with dementia focuses on their remaining strengths, their long-held preferences, and what still brings them comfort, rather than a generic script. Expert insight

“The biggest mistake I see in this industry is agencies that build a care plan around a diagnosis instead of a person. Two clients with the exact same condition can need completely different approaches, one wants independence protected at all costs, another wants more hands-on support so they can conserve their energy for what matters to them. Our job is to figure out which one your loved one actually is, not assume.”

Frequently Asked Questions

What does “patient-centered” care actually mean?

It means the care plan is built around your loved one as an individual, their routine, preferences, and comfort, rather than a fixed checklist applied the same way to every client.

How is this different from a standard home care plan?

A standard plan often starts with a list of tasks to complete. A patient centered plan starts by understanding the person first, then chooses the services and schedule that actually fit them.

Does my loved one get a say in their own care decisions?

Yes, that’s the whole point. Patient centered care means your loved one keeps as much choice and control over their daily life as possible, with support built around those choices.

Can the plan change as my parent’s needs change?

Yes. We revisit the plan regularly, with your loved one involved in that conversation, and adjust services and hours as their situation shifts.

Does long-term care insurance cover patient centered care services?

Many policies do, once daily activity benefit triggers are met. We help review your long-term care insurance policy and manage the claims paperwork directly.

Dignity and Respect

Health care practitioners listen to and honor patient and family perspectives and choices. Patient and family knowledge, values, beliefs, and cultural backgrounds are incorporated into the planning and delivery of care. This may include making special accommodations for religious restrictions regarding meal preparation and other similar considerations when applied to home care or other care situations. Operating this way helps to create a relationship in which all parties are mutually respectful and respected.

Patient-Centered Care — It's What We Do Best

Patient and family-centred care is an innovative approach to the planning, delivery and evaluation of health care. This facilitates mutually beneficial partnerships among health care providers, patients and families. Patient- and family-centred care applies to people of all ages, and it may be practiced in any setting.

Information Sharing

Caregivers communicate and share complete and unbiased information with patients and families in ways that are affirming and useful. Patients and families receive timely, complete, and accurate information in order to effectively participate in care and decision-making. When home care services are used to assist with care, caregivers can provide additional insights that can help clients to best manage conditions and enjoy the highest quality of life.

Participation

Clients and families are encouraged and supported in participating in care and decision making at the level they choose. When ComForCare caregivers assist with client care, caregivers work closely with clients and family members to determine scheduling and care needs. Family members can take as little or as much control over caregiving as desired, with ComForCare caregivers filling in the gaps or taking charge as needed. Meeting client needs and wishes is a priority. Read more about respite care for family caregivers.

Collaboration

Patients and families are also included on an institution-wide basis. Health care leaders collaborate with patients and families in policy and program development, implementation and evaluation. Patients’ and families’ suggestions and insights are also considered when making changes to health care facility design and in professional education, along with the delivery of care. ComForCare uses patient and family suggestions, as well as patients’ needs evaluations, to determine how best to provide care for each individual. This information is also ultimately used to improve care for all patients in the area. Patients may be given more companionship time, have more attention focused on condition management, or other changes may be made to care routines as determined through collaboration with patients and families.
