One-Time Hospital-to-Home Care Services

New routines, regiments, and risks can be overwhelming for those who experience it for the first time. Thankfully, our staff members are experts in safely guiding clients on the next step of their healing journey.

When your loved one begins a transition of care, our team will meet with medical personnel, review discharge procedures, pick up prescriptions, and safely get your loved one home. Caregivers will then review the discharge plan with your family, answering any questions you may have.

Hospital-to-Home service includes:

  • An experienced, reliable caregiver who meets your loved one in the hospital room or discharge area
  • Transportation home with stops for picking up prescriptions or groceries
  • Help understanding and following discharge plans
  • A debriefing phone call to the family (or doctor or health coach)
  • Scheduling and attending follow up appointments
  • Meal preparation
  • Light housekeeping and laundry
  • Identify trip hazards and make home safety recommendations
  • Educate the client and family members on red flags or warning signs for chronic conditions
  • Bathing, grooming, and personal hygiene

Short-Term Care Services

What Is Transitional Care?

Transitional care is when someone moves from one care setting – such as a hospital, skilled nursing facility, rehabilitation center or even their home – to another. Unfortunately, these transitions don’t always go smoothly and a person can be readmitted to the hospital.

Transitional care services from ComForCare Home Care are available 24 hours a day, seven days a week - including holidays. Services vary by location.

Transition to Home Package

For individuals that need Transitional Care services, ComForCare offers a one-time “Transition to Home” package. This includes assistance from the hospital, rehabilitation center or skilled nursing facility along with help getting settled in at home. We’ll even call a friend or family member to let them know their loved one has arrived home.

Preparing for Discharge

As you or a loved one prepare for discharge, call us for a no obligation consultation. We can provide as much or as little support needed to help someone safely transition home. Our caregivers are available 24/7 including holidays, and we customize our care based on each client’s needs and preferences.
