I have seen how much of a difference a little preparation and the right support can make during those first days at home.<\/p>\n\n\n\n<p>I work with families who want their loved ones to recover safely and comfortably at home. In this guide, I\u2019ll walk you through a practical checklist for getting the home ready, staying on top of medications and appointments, watching for changes, and knowing when it may be time to ask for extra help.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-why-the-first-two-weeks-matter-most\">Why the first two weeks matter most<\/h2>\n\n\n\n<p>Coming home is not the finish line. It is the start of what professionals call the care transition, which just means the handoff from hospital care to care at home. This is where small things slip through the cracks. A pill gets skipped. A follow up appointment never gets booked. A rug that has been fine for twenty years suddenly becomes a <a href=\"https:\/\/www.comforcare.com\/california\/san-diego\/programs\/gaitway-fall-prevention\/\">trip hazard<\/a>. Keep a copy somewhere you can find it at 2 a.m.<\/li>\n\n\n\n<li><strong>Get a real contact.<\/strong> A name and a direct number beat the main hospital line every time.<\/li>\n\n\n\n<li><strong>Go over the medication list.<\/strong> Ask what is new, what stopped, and what stays the same.<\/li>\n\n\n\n<li><strong>Book follow up appointments right away.<\/strong> Openings disappear fast.<\/li>\n\n\n\n<li><strong>Ask about equipment.<\/strong> Walkers, shower chairs, and raised toilet seats are called durable medical equipment. Ask which ones are recommended and how to get them.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-getting-the-house-ready\">Getting the house ready<\/h2>\n\n\n\n<p>My favorite trick is to walk through the home pretending I have a sore knee and a tired brain. You will spot things you stopped noticing years ago.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Clear walkways of cords, clutter, and loose rugs<\/li>\n\n\n\n<li>Add night lights between the bedroom and bathroom<\/li>\n\n\n\n<li>Move everyday items to waist height so nobody has to stretch or stoop<\/li>\n\n\n\n<li>Make sure the bathroom has grab bars or something sturdy to hold<\/li>\n\n\n\n<li>Stock the kitchen with easy, nourishing food<\/li>\n\n\n\n<li>Set up a comfy resting spot close to the bathroom<\/li>\n<\/ul>\n\n\n\n<p>Many homes in Mission Hills, Kensington, and University Heights have gorgeous character and equally gorgeous staircases. Charming, yes. Recovery friendly, not always. Give those spots extra attention.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-medications-without-the-meltdown\">Medications without the meltdown<\/h2>\n\n\n\n<p>Medication mix ups are the number one worry I hear about, and they are also among the most preventable. Ask the pharmacist for a medication reconciliation. That is a fancy phrase for comparing the old medication list with the new one so nothing doubles up or goes missing.<\/p>\n\n\n\n<p>Then keep it simple:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Use a weekly pill organizer<\/li>\n\n\n\n<li>Post one written list on the fridge with names, doses, and times<\/li>\n\n\n\n<li>Bring that list to every appointment<\/li>\n\n\n\n<li>Set phone alarms for dose times<\/li>\n<\/ul>\n\n\n\n<p>If your loved one lives alone, or you cannot be there at dose time, this is a great place to bring in a <a href=\"https:\/\/www.comforcare.com\/california\/san-diego\/in-home-care\/personal-in-home-care\/\">helping hand<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-warning-signs-worth-a-phone-call\">Warning signs worth a phone call<\/h2>\n\n\n\n<p>You do not need a medical degree to sense that something is off. After a hospital stay, even these can wipe someone out for a while.<\/p>\n\n\n\n<p>Ask yourself, honestly:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Can they shower safely on their own?<\/li>\n\n\n\n<li>Are they eating real meals, not just crackers?<\/li>\n\n\n\n<li>Can they get to their appointments?<\/li>\n\n\n\n<li>Is someone checking in every day?<\/li>\n<\/ul>\n\n\n\n<p>If you said &#8220;not sure&#8221; to any of those, that is your answer. Families in La Jolla Shores, Bankers Hill, and North Park often tell me that even a few hours of support each week lets everyone breathe again.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-do-not-skip-the-feelings\">Do not skip the feelings<\/h2>\n\n\n\n<p>A hospital stay is scary. Many seniors come home shaken, a little embarrassed, and frustrated about what they cannot do yet. Some small things go a long way:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Let them make choices, even tiny ones, like dinner or the TV channel<\/li>\n\n\n\n<li>Keep familiar routines whenever possible<\/li>\n\n\n\n<li>Invite a friend over for tea or a short stroll when they are ready<\/li>\n\n\n\n<li>Say out loud that healing takes time<\/li>\n<\/ul>\n\n\n\n<p>Independence and dignity are tied together. Protect both wherever it is safe to do so.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-quick-family-checklist\">The quick family checklist<\/h2>\n\n\n\n<p>Screenshot this one. I won&#8217;t tell anyone.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Get the discharge summary and a contact name<\/li>\n\n\n\n<li>Review and organize all medications<\/li>\n\n\n\n<li>Book follow up appointments<\/li>\n\n\n\n<li>Arrange needed equipment<\/li>\n\n\n\n<li>Make the home safer<\/li>\n\n\n\n<li>Stock easy, healthy food<\/li>\n\n\n\n<li>Decide who checks in each day<\/li>\n\n\n\n<li>Learn the warning signs<\/li>\n\n\n\n<li>Line up extra help before you are exhausted<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-one-last-thing-from-me\">One last thing from me<\/h2>\n\n\n\n<p>You do not have to get this perfect.
