Her mother can walk, mostly, and doesn&#8217;t need a ventilator or a feeding tube. She just can&#8217;t be by herself right now.<\/p>\n\n\n\n<p>If that&#8217;s you, take a breath. You&#8217;re not failing anyone. You&#8217;ve just run into a gap that almost nobody explains to you ahead of time, and I want to explain it now.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-medically-cleared-actually-means\">What &#8220;Medically Cleared&#8221; Actually Means<\/h2>\n\n\n\n<p>Here&#8217;s the translation nobody gives you in the discharge meeting. \u201cMedically cleared\u201d means the hospital or rehab facility has completed the care needed for medical recovery. A skilled nursing facility, or SNF, provides short term care and support during recovery. Your mother&#8217;s blood pressure is stable. Her wound is healing. There&#8217;s no acute medical crisis keeping her there.<\/p>\n\n\n\n<p>It does not mean she&#8217;s ready to cook dinner, climb into the shower safely, remember her medications, or get to the bathroom at 3am without falling.<\/p>\n\n\n\n<p>The system often treats those two things as the same problem, but they are not. That mismatch is exactly where families get stuck.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-real-problem-isn-t-medical-it-s-coverage\">The Real Problem Isn&#8217;t Medical. It&#8217;s Coverage.<\/h2>\n\n\n\n<p>Once someone no longer needs facility level care, the facility has completed the care it is responsible for providing. What she needs now is supervision, help with daily tasks, and someone paying attention. That&#8217;s not a medical need in the clinical sense. It&#8217;s a human need. Bathing, dressing, meal prep, medication reminders, mobility help, companionship, transportation to appointments. It&#8217;s not about treating an illness. It&#8217;s about making daily life safe and manageable.<\/p>\n\n\n\n<p>Most families in your situation, where the hospital says there&#8217;s no medical reason to keep her, actually need home care, not home health.
