Thirty-seven adults with intellectual disabilities completed comprehension assessments using both original and adapted versions. The adapted texts most often maintained comprehension rather than producing a consistent improvement or decline. Some combinations of readers and texts showed improvement, while others showed worsening. Participants reported fewer syntactic difficulties with the adapted versions, but vocabulary challenges remained.

The practical takeaway is important: accessible communication should be individualized and evaluated with the person who will use it.

What the Study Can and Cannot Tell Us

This peer-reviewed study used repeated assessments across readers and texts. It provides useful evidence that simplifying information does not work the same way for everyone.

However, the study focused on Spanish narrative materials, not health instructions, Regional Center notices, Individual Program Plans, or employment documents. We should not assume the exact findings apply to every communication setting.

The study also does not mean Easy-to-Read materials are unhelpful. Shorter sentences, clearer organization and less unnecessary jargon can make information easier to approach.

The larger point is that understanding involves more than sentence length. Vocabulary, prior knowledge, attention, memory, decoding skills and the purpose of the task can all affect how someone understands information.

A document can be shorter and still contain unfamiliar words. Pictures can sometimes distract rather than clarify. And removing too much detail can make it harder for someone to understand their choices, rights, responsibilities, or risks.

Start With the Person and the Purpose

Before rewriting information, consider what the person needs to do with it.

Are they:

Choosing between options?

Preparing for an appointment?

Reviewing a spending plan?

Learning a household routine?

Understanding a new service?

Making an important personal decision?

The information that matters most depends on the decision or task.

It is also helpful to ask about communication preferences directly. Some people prefer identity-first language, while others prefer person-first language. Some may want information read aloud. Others may prefer to read privately and ask questions afterward.

A person may use pictures, large print, high contrast, symbols, a communication device, captions, a demonstration, or a trusted supporter. These preferences may also change depending on the topic or setting.

Five Ways to Make Information More Usable

1. Use familiar words and explain necessary technical terms

Everyday language can make information easier to understand. When technical terms are necessary, explain them in plain language.

For example, if an official notice uses a term such as “vendorization,” explain what the term means and why it matters to the person.

2. Keep sentences focused but don’t remove important information

Try to communicate one main idea at a time while preserving essential details.

Do not simplify away important information about choices, costs, deadlines, risks, responsibilities, or rights.

3. Organize information around what the person needs to know

A useful structure might be:

What happened? → What does it mean? → Do I need to do something? → Who can help?

Putting information in a practical order can make it easier to follow and act on.

4. Offer different formats when possible

Written information does not have to stand alone.

Depending on the person’s preferences, information might be paired with:

A spoken explanation

Relevant pictures or symbols

A demonstration

A short video

A checklist

Captions or transcripts

Large-print materials

The goal is to match the support to the person not to rely on assumptions about what someone with a disability will need.

5. Check understanding respectfully

Checking understanding should not feel like a test.

Try questions such as:

“What feels clear so far?”

“Would you like me to explain that another way?”

“Would you like to tell me what you plan to do next?”

A teach-back approach can help identify what needs clarification while also showing whether the communicator explained the information effectively.

The Role of Supporters

Family members, direct support professionals and other trusted people can help someone understand complex information. However, support should not replace the person’s voice.

Whenever possible, speak directly to the person first. Allow enough time for processing and avoid answering questions on their behalf unless that is the person’s stated preference or part of their support needs.

Supported decision-making can involve reviewing information over several conversations, comparing a manageable number of meaningful options and documenting the person’s priorities.

The goal is not to make the decision for someone. The goal is to provide the support they need to understand information, communicate their preferences and act on their choices.

A Communication Check for Home and Community Services

When introducing a new routine, service, schedule, or activity, consider these questions:

Is the purpose clear?

Are important choices and rights still included?

Are unfamiliar words explained?

Does the format match the person’s communication preferences?

Has the person had enough time to process the information?

Has the person had a meaningful opportunity to respond?

These simple questions can help make everyday communication more respectful and useful.

Supporting Person-Centered Communication at Home

At ComForCare Home Care North San Diego, we believe understanding a person’s communication preferences is an important part of providing respectful, person-centered support.
