But better risk estimates are not the same as certainty about an individual’s future.

On October 7, 2026, NIH Research Matters highlighted a study examining two sources of information associated with Alzheimer’s disease and cognitive impairment: a blood biomarker called phosphorylated tau 217, or p-tau217, and variants of the APOE gene.

The research team combined data from seven prospective studies involving more than 8,500 older adults in Canada, the Dominican Republic, and the United States. Some participants were cognitively healthy at the beginning of the studies, while others were at risk for or already living with cognitive impairment. Data collection covered the years 1992 through 2025.

The researchers found that combining p-tau217 levels with APOE status improved estimates of cognitive-impairment risk and timing in the study populations.

That is an important research advance. It does not mean that a blood test and genetic result can tell an individual exactly whether or when dementia will develop.

What p-tau217 and APOE Represent

P-tau217 is a form of tau protein that can be measured in blood. Higher levels can reflect biological changes associated with Alzheimer’s disease. However, results need to be interpreted in the context of the specific test, the individual’s clinical situation and the population in which the test has been studied.

APOE is a gene involved in lipid transport. One variant, APOE-e4, is associated with an increased risk of Alzheimer’s disease.

But APOE-e4 is neither necessary nor sufficient for developing Alzheimer’s.

Some people who carry APOE-e4 never develop Alzheimer’s, while many people who develop Alzheimer’s do not carry the variant.

In this study, people with higher p-tau217 levels and APOE-e4 were particularly likely to have or later develop cognitive impairment. The researchers also found that the time to cognitive impairment shortened more quickly as p-tau217 levels increased among APOE-e4 carriers than among noncarriers.

These findings can help researchers understand patterns of risk. They should not be interpreted as an individual prediction.

The Findings Differed Across Groups

One important consideration was that the associations were stronger among non-Hispanic White participants than among people from other racial or ethnic groups.

NIH noted that previous research has found that the risk associated with APOE-e4 can vary with ancestry.

This matters when researchers develop predictive tools. A model may perform differently in populations that were not adequately represented in the research used to develop or validate it.

More representative studies and additional validation are important before risk-prediction tools are applied broadly.

For clinicians, this also reinforces the importance of looking at the whole person. Biomarker results and genetic information are only pieces of a much larger clinical picture.

Why a Risk Estimate Is Not a Diagnosis

Dementia describes a decline in thinking abilities that interferes with everyday life. Memory and thinking changes can have many possible causes.

Medication effects, depression, sleep disorders, thyroid problems, infections, hearing loss and other neurological or medical conditions can affect cognition.

A medical evaluation may include a health history, physical and neurological examination, cognitive testing, laboratory testing and brain imaging when appropriate.

Biomarkers may eventually provide additional information within that process, but they do not replace a comprehensive evaluation.

Genetic testing also deserves careful consideration. Learning about APOE status can raise questions about family members, emotional well-being, privacy and other personal concerns. Anyone considering APOE or other genetic testing should discuss the potential benefits, limitations and implications with a qualified healthcare professional or genetic counselor.

What Families Can Do Now

Families do not need to seek a biomarker test simply because of one research report.

A practical first step is to pay attention to changes in everyday life.

If you notice concerns about memory, organization, judgment, or other thinking abilities, keep a simple record of:

What happened

When the change began

How often it occurs

Whether it is getting worse

Whether it affects medications, finances, driving, cooking, appointments, or other daily activities

Bring those observations and an updated medication list to a medical appointment. Specific examples from everyday life can help healthcare professionals understand what is happening outside the clinical setting.

Sudden confusion or an abrupt change in thinking is different from a gradual change and can signal an urgent medical problem. Seek prompt medical attention when this occurs.

Supporting Someone While Questions Are Being Explored

A family does not have to wait for a diagnosis before creating a supportive environment.

Simple routines, clear communication, familiar surroundings, and appropriate assistance with daily activities can help reduce stress while a family works with healthcare professionals to understand changes in cognition.

Home care can also provide another set of eyes on everyday routines and help families notice changes that may be useful to discuss with a healthcare professional.

ComForCare Home Care North San Diego can support individuals and families with daily routines, personal care, companionship and other non-medical needs.
