Family caregivers may feel pressure to correct the facts, complete a task, or stop a behavior as quickly as possible.

But dementia care often calls for a different first step: pause, slow the moment down, and become curious about what the person may be communicating.

Studies published in August 2026 examined a structured education program for family caregivers called Watch-Assess-Need Intervention-Think, or WANT. The name matters less than the sequence. The approach encourages caregivers to observe what is happening, consider possible causes, respond to the person’s needs, and reflect on what happened afterward.

What the Study Found

Researchers recruited 90 family caregivers of people living with dementia from communities in Taiwan. Participants were randomly assigned to either a one-month WANT education program or a control group that continued its usual routine.

Researchers used questionnaires to assess dementia-care competence and self-efficacy before and after the program and also interviewed participants about their experiences.

The caregivers who received the WANT training showed improvements in knowledge, caregiving skills, and self-efficacy. Changes in attitudes were smaller. During interviews, participants described learning new ways to understand behavioral expressions, regulate their own responses, and strengthen relationships with family members living with dementia.

The findings are encouraging, but they also need to be interpreted carefully. The study was relatively small, took place in one country, and evaluated outcomes over a short period. It cannot tell us whether every family would experience the same benefits or whether a brief education program leads to long-term health improvements.

Instead, the study supports the continued use and evaluation of structured caregiver education. It does not provide a universal script for every dementia-care situation.

A Five-Step Pause for Everyday Dementia Care

The WANT approach can be thought of as a simple pause before reacting.

1. Watch

Start by noticing what happened before the change in behavior.

Was the room noisy? Did someone new arrive? Was a task rushed? Did the person appear tired or uncomfortable?

Pay attention to facial expressions, posture, tone of voice, and other signs that may provide clues about what the person is experiencing.

2. Assess

Consider what might be contributing to the person’s response.

Common possibilities include:

Pain or physical discomfort

Hunger or thirst

Fatigue

Temperature

Too much noise or stimulation

Changes in lighting or surroundings

Fear or confusion

Loneliness

Difficulty understanding instructions

Caregivers are not expected to diagnose a medical condition. The goal is to notice possible triggers and determine whether additional professional support may be needed.

Identify the Need

Instead of asking only, “How do I stop this behavior?”, try asking:

“What might this person need right now?”

The answer could be reassurance, privacy, a quieter environment, a bathroom break, a familiar activity, physical comfort, or simply more time.

This shift can help move the focus from controlling behavior to understanding the person.

4. Intervene Simply

Try one respectful change at a time.

You might:

Lower your voice.

Reduce background noise.

Offer two simple choices.

Give the person additional time to respond.

Acknowledge the person’s feelings rather than arguing about details.

Step away and return later if the situation is safe.

A calm, simple response can sometimes be more effective than trying to explain or correct everything at once.

5. Think

After the moment has passed, take a few minutes to reflect.

What happened beforehand? What seemed to make the situation better or worse? What worked?

Keeping a simple record can help identify patterns over time and provide useful information to family members, caregivers, and the person’s healthcare team.

When a Change in Behavior May Need Medical Attention

Not every change in behavior should be attributed to dementia.

Sudden confusion or a rapid change in behavior can have other causes, including pain, infection, dehydration, medication effects, poor sleep, constipation, or other health concerns.

Contact a qualified healthcare professional promptly about new, severe, or concerning changes. If there is immediate danger or a medical emergency, call emergency services.

Supporting the Caregiver Is Part of Person-Centered Care

Person-centered dementia care includes the family caregiver, too.

Taking a pause is much harder when someone is exhausted, overwhelmed, or trying to manage everything alone. Families can help reduce that burden by sharing responsibilities, scheduling regular breaks, seeking dementia-specific education, and keeping a shared record of approaches that work well.

No caregiver should be expected to handle every difficult moment alone.

Non-medical in-home care may also provide practical support with companionship, personal care, meal preparation, everyday routines, and observation within an established care plan. This type of support does not replace medical evaluation or treatment. Instead, it can complement an existing plan and help families maintain greater consistency at home.

The goal is to keep communication open among the person living with dementia, family members, caregivers, and qualified healthcare professionals.

A Pause Can Change the Moment

Dementia can make communication more difficult, but behavior can also provide important clues.

Before correcting, arguing, or rushing to stop a behavior, consider taking a moment to watch, assess, identify the need, intervene simply, and think about what happened.

Sometimes, the most helpful response begins with slowing down and listening to what the person may be trying to communicate.

Medical Information Disclaimer

This article is intended for general educational purposes only and does not provide diagnosis or medical advice.
