MEDICATION MANAGEMENT<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Parkinson\u2019s medications must be taken exactly on schedule.<\/li>\n\n\n\n<li>Late or missed doses can cause \u201cOFF\u201d time: more tremor, slowness, and stiffness.<\/li>\n\n\n\n<li>Dyskinesias are extra, involuntary movements that can happen when medication levels are high.<\/li>\n\n\n\n<li>High-protein meals may reduce levodopa absorption in some people.<\/li>\n\n\n\n<li>Track patterns: Are symptoms worse right before the next dose? Write this down and report it to the MD.<\/li>\n<\/ul>\n\n\n\n<p><strong>II. MOVEMENT &amp; EXERCISE<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Exercise acts like medicine for Parkinson\u2019s disease.<\/li>\n\n\n\n<li>It improves gait, balance, mobility, confidence, and emotional wellbeing.<\/li>\n\n\n\n<li>Best results come when exercise is guided by a physical therapist (PT) familiar with PD.<\/li>\n\n\n\n<li>Use safe strategies:<\/li>\n\n\n\n<li>Clear hallways and remove clutter.<\/li>\n\n\n\n<li>Use gait belts when appropriate.<\/li>\n\n\n\n<li>Use mobility devices (cane, walker, wheelchair) as recommended.<\/li>\n<\/ul>\n\n\n\n<p><strong>III. NUTRITION<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Constipation is very common. A combination of fiber + fluids usually helps.<\/li>\n\n\n\n<li>Swallowing difficulty can develop as the disease progresses.<\/li>\n\n\n\n<li>Request a speech therapy evaluation if you notice coughing or choking during meals.<\/li>\n\n\n\n<li>Talk with the MD about protein timing if medications are not lasting as long or \u201cwear off\u201d too soon.<\/li>\n<\/ul>\n\n\n\n<p><strong>IV. EMOTIONAL SUPPORT<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Depression, anxiety, apathy, and cognitive slowing are common in Parkinson\u2019s.<\/li>\n\n\n\n<li>Validate feelings and avoid minimizing symptoms (\u201cYou\u2019re fine,\u201d \u201cIt\u2019s nothing\u201d).<\/li>\n\n\n\n<li>Encourage:<\/li>\n\n\n\n<li>Social connection (visits, phone calls, support groups)<\/li>\n\n\n\n<li>Routines and hobbies that the person still enjoys<\/li>\n\n\n\n<li>Patience and calm communication from all caregivers<\/li>\n\n\n\n<li>Report suicidal thinking, major confusion, or new hallucinations to the MD right away.<\/li>\n<\/ul>\n\n\n\n<p><strong>STAGE-BASED GUIDANCE<\/strong><\/p>\n\n\n\n<p><strong>A. EARLY STAGE<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mild symptoms, usually on one side first.<\/li>\n\n\n\n<li>Medication response is generally good.<\/li>\n\n\n\n<li>Encourage regular exercise and safe movement habits.<\/li>\n\n\n\n<li>Monitor:<\/li>\n\n\n\n<li>Sleep<\/li>\n\n\n\n<li>Mood<\/li>\n\n\n\n<li>Constipation<\/li>\n\n\n\n<li>Driving safety<\/li>\n\n\n\n<li>Work and daily activity changes<\/li>\n<\/ul>\n\n\n\n<p><strong>B. MID STAGE<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Symptoms are now on both sides.<\/li>\n\n\n\n<li>Freezing of gait and falls are more frequent.<\/li>\n\n\n\n<li>\u201cWearing off\u201d between doses and possible dyskinesias.<\/li>\n\n\n\n<li>Home modifications:<\/li>\n\n\n\n<li>Remove loose rugs<\/li>\n\n\n\n<li>Improve lighting<\/li>\n\n\n\n<li>Install grab bars<\/li>\n\n\n\n<li>Consider a raised toilet seat and shower chair<\/li>\n<\/ul>\n\n\n\n<p>Therapies:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>PT for balance and gait<\/li>\n\n\n\n<li>OT for home safety and adaptive equipment<\/li>\n\n\n\n<li>Speech therapy for low voice and swallowing issues<\/li>\n\n\n\n<li>Call the MD if you notice:<\/li>\n\n\n\n<li>New or frequent choking or coughing with food<\/li>\n\n\n\n<li>Frequent falls<\/li>\n\n\n\n<li>Big behavior changes or new confusion<\/li>\n<\/ul>\n\n\n\n<p><strong>C. LATE STAGE<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>High risk of falls; severe slowness and stiffness.<\/li>\n\n\n\n<li>May need a walker or wheelchair and help with most daily activities.<\/li>\n\n\n\n<li>Dizziness or fainting when standing (orthostatic hypotension).<\/li>\n\n\n\n<li>More cognitive changes and possible hallucinations or delusions.<\/li>\n\n\n\n<li>Significant swallowing difficulty, weight loss, or frequent infections.<\/li>\n<\/ul>\n\n\n\n<p>Consider:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Palliative care for symptom relief and caregiver support<\/li>\n\n\n\n<li>Hospice discussions based on overall decline and goals of care<\/li>\n<\/ul>\n\n\n\n<p><strong>NON-MOTOR SYMPTOMS<\/strong><\/p>\n\n\n\n<p><strong>(OFTEN EARLY)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Sleep problems<\/li>\n\n\n\n<li>Constipation<\/li>\n\n\n\n<li>Reduced sense of smell<\/li>\n\n\n\n<li>Pain and fatigue<\/li>\n\n\n\n<li>Slower thinking and memory changes<\/li>\n\n\n\n<li>These can appear years before obvious tremor or stiffness. Track changes\u2014people may feel embarrassed to bring them up.<\/li>\n<\/ul>\n\n\n\n<p><strong>MOTOR SYMPTOMS<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Tremor at rest<\/li>\n\n\n\n<li>Rigidity (stiffness)<\/li>\n\n\n\n<li>Slow movement (bradykinesia)<\/li>\n\n\n\n<li>Shuffling gait<\/li>\n\n\n\n<li>Freezing episodes<\/li>\n\n\n\n<li>Reduced arm swing<\/li>\n\n\n\n<li>Masked facial expression (reduced facial movement)<\/li>\n<\/ul>\n\n\n\n<p><strong>KEY SAFETY REMINDERS<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Medication timing = function. Small delays can make a big difference.<\/li>\n\n\n\n<li>Encourage daily exercise within the person\u2019s abilities.<\/li>\n\n\n\n<li>Monitor nutrition, hydration, and bowel health.<\/li>\n\n\n\n<li>Always validate feelings and protect dignity.<\/li>\n\n\n\n<li>Report major changes (falls, choking, sudden confusion, severe mood changes) to the medical team promptly.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Parkinson&#8217;s Pathway notes to help with understanding and the training.
