An In-Home evaluation starts with a nurse or health care professional performing a comprehensive evaluation of your loved one's physical and emotional condition. A plan is developed, and a caregiver is matched based on personality and interests. Care and coordination are part of the next step, during which time plans for medication, appointments, insurance forms, and other administrative duties help solidify the details of care. The final step is ongoing coordinated care, along with frequent and regular communication with the family which results in successful senior and elder care management for your loved one.
